Follow the on-screen instructions to fully complete and submit it with all the requested information. After having submitted the form, you will obtain one entry into the Sweepstakes and one Instant Win Game.
Entries to both the Sweepstakes and the Instant Win Game are limit to one per person per each day of the promotion period, which ends on July 31st, 2019. The Grand Prize and the First Prize winners will be selected on August 1st, 2019 in a random drawing from among all eligible entries received. Winner will be notified via email. Instant Win Game winners will be notified on-screen immediately after the game has been completed.
