To enter this Sweepstakes, you must visit your nearest Melting Pot and make sure you keep the receipt. The receipt will contain a code that you must submit on fonduesurvey.com along with the online entry form. You will also be required to complete a survey. After having completed these steps you will obtain one entry into the Sweepstakes.
For each submitted receipt you will obtain one entry into the Sweepstakes. Throughout the entire Sweepstakes Period, winners will be selected in random drawings from among all eligible entries received. Winners will be notified via the contact information provided at the time of entry. Odds of winning depend on the number of eligible entries received.
