To enter this Sweepstakes, go to the official website during the entry period, and, once there, scroll down to locate the online entry form. Then, follow the on-screen directions to fully complete and submit it with all the requested information. After having completed these steps, you will obtain one entry into the Sweepstakes.
Entries are limit to one per person per each day of the Sweepstakes Period, which ends on November 12th, 2021. On each week of the Sweepstakes closure, winners will be selected in a random drawing from among all the eligible entries received. Winning notification will be conducted via email. Odds of winning depend on the number of eligible entries received.
The Grand Prize winner will obtain a trip package, up to $40,000, includes round trip, coach-class air transportation for Grand Prize winner and up to five travel companions from a major airport near winner’s home (determined by Sponsor in its sole discretion) to a city determined by winner; accommodations at a hotel determined by Sponsor in its sole discretion; ground transportation to and from destination airport and hotel (if applicable); various activities depending
on selected city; and travel agent services. The Grand Prize winner will also be provided with a single $10,000 cash payment in the form of a corporate check, which is intended to assist the winner with any federal, state, or other tax obligations related to the Grand Prize.
The First Prize winner will be awarded an Extreme Entertainment Makeover Prize Pack will be awarded as $30,000 in Sam’s Club Gift Cards.
Second Prize winner will be awarded a Lifetime Plus Sam’s Club membership will be awarded as $4,500.00 in Sam’s Club gift cards.
